An exciting full-time opportunity is available for a Medical Director to lead an innovative Program of All-Inclusive Care for the Elderly (PACE) in West Springfield, MA. This leadership position focuses on providing compassionate, coordinated care that helps seniors with chronic conditions maintain independence and quality of life in their communities.
Key Responsibilities:
• Provide medical direction and oversight of participant care and clinical outcomes.
• Supervise Primary Care Providers and Advanced Practice Providers.
• Participate in interdisciplinary team meetings and care planning.
• Conduct assessments, develop treatment plans, and manage ongoing care.
• Oversee quality improvement and compliance initiatives (QAPI).
Qualifications:
• Active physician license in Massachusetts (required).
• Board Certification in Internal Medicine or Family Practice (required).
• Geriatric Medicine certification or experience (preferred).
• DEA and CDS certification (required).
• At least one year of experience working with a frail or elderly population.
• Previous supervisory and leadership experience.
