We are looking for Full-Time Homecare Physical Therapists and Occupational Therapists!

Needs are in designated areas of the Capital District as well as designated areas of WARREN/WASHINGTON Counties!

Our Therapists are responsible for managing a caseload of patients requiring home-based therapy, primarily post-acute or subacute. Enjoy starting and ending your day from home!

What you will do:

• Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
• Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
• Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)
• Experience the rewards of appreciation from being able to address the whole patient
• Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
• Minimal weekend and holiday rotations

What you will need:

• Current NYS PT or OT license and registration
• Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required
• Home care experience preferred, but not required
• Valid Driver's License and Reliable Vehicle. This position provides mileage reimbursement.

Physical Therapist Pay Range: $37.60 - $48.10

Occupational Therapist Pay Range: $35.85 - $45.96

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
